Oregon House Bill 2656 requires Oregon public school districts to offer the Oregon Student Health Survey to students annually in grades 6, 8, and 11. The SHS is a comprehensive survey that asks about the whole child to get a more complete picture of students' health.
The Oregon Student Health Survey is anonymous and administered online. It generally completed in our district in December.
Opt-Out:
If you want your student not to participate, please complete and submit this notification form in English or Spanish at least 5 days before the survey. A reminder will be sent to parent's informing them of the survey dates and providing access to the opt-out form. You only need to complete the form if you want your student not to participate.
